Can You Switch from Medicare Advantage to a Medicare Supplement in North Carolina?

Plenty of people pick a Medicare Advantage plan, use it for a year or two, and then wonder whether a Medicare Supplement plan would suit them better. If that is you, the good news is that switching is possible. The catch is that it is not as simple as signing up, and the timing of your move matters more than most people expect.

Yes, but timing and health questions matter

Yes, you can switch from a Medicare Advantage plan to a Medicare Supplement, also called Medigap, plan in North Carolina. Two things decide how smoothly it goes: you can only leave Medicare Advantage during certain windows, and in most cases a Medigap insurer can ask health questions and decide whether to accept you. Timing and your health history both shape the outcome.

If you are still deciding between the two in the first place, our guide to Medicare Advantage vs Medicare Supplement in North Carolina compares them side by side. This post is about making the move once you have decided to leave Advantage.

When can you leave Medicare Advantage

You cannot drop a Medicare Advantage plan whenever you like. Two windows each year let you move back to Original Medicare. The Annual Enrollment Period, October 15 to December 7, lets you switch for coverage that starts January 1. The Medicare Advantage Open Enrollment Period, January 1 to March 31, gives you one more chance if you are already in an Advantage plan.

A few life events, such as moving out of your plan’s service area or your plan leaving Medicare, can also open a Special Enrollment Period outside those windows. For the full picture of the fall window, see our post on the Medicare Annual Enrollment Period. Keep in mind that leaving Advantage is only half the move. Getting a Medigap policy is the other half, and it follows different rules.

How medical underwriting works for Medicare Supplement plans

Here is the part that surprises people. Leaving Medicare Advantage returns you to Original Medicare, but it does not guarantee you a Medigap policy. Outside certain protected situations, a Medigap insurer in North Carolina can ask about your health, charge more based on it, or turn you down. This health review is called medical underwriting.

The one time you are guaranteed a Medigap policy with no health questions is your six-month Medigap open enrollment, which starts when you are 65 or older and enrolled in Part B. If that window has passed, underwriting usually applies unless you qualify for a guaranteed issue right, which is covered next. You can apply for Medigap at any time of year, but outside a protected window, acceptance is not guaranteed.

Guaranteed issue rights that skip health questions

In some situations, federal rules give you a guaranteed issue right, which means a Medigap insurer must sell you a policy without health questions and cannot charge you more because of your health. The one most Advantage members ask about is the trial right.

The trial right applies if you joined Medicare Advantage when you first became eligible for Medicare, or if you dropped a Medigap policy to try Advantage for the first time. If you switch back to Original Medicare within your first 12 months, you can buy a Medigap policy on a guaranteed issue basis. A few other events also trigger a guaranteed issue right, such as your Advantage plan leaving Medicare or you’re moving out of its service area.

These rights come with strict deadlines, usually a 63-day window after your Advantage coverage ends, so acting early matters. The trial right also applies only the first time you join Medicare Advantage. A licensed agent can confirm whether your situation qualifies and how a specific insurer would handle your application.

Adding a Part D drug plan when you switch

Most Medicare Advantage plans include prescription drug coverage, but Medicare Supplement plans do not. So, when you move to Original Medicare and a Medigap policy, you also need to join a standalone Part D prescription drug plan, both to keep your medications covered and to avoid a late enrollment penalty.

Sign up for Part D during the same window you use to leave Advantage, so your drug coverage starts the same day as the rest of your new coverage. Our overview of Medicare Part D explains how the drug benefit works.

Steps to switch without a coverage gap

The safest way to switch is to line up your new coverage before you drop the old. Because a Medigap insurer can decline you outside a guaranteed issue window, the order of these steps matters.

Before you cancel anything, work through these steps in order:

  1. Confirm your window. Check whether you are in the Annual Enrollment Period, the Advantage Open Enrollment Period, your 12-month trial right, or another guaranteed issue situation.
  2. Apply for the Medigap policy first, and get your acceptance confirmed before you cancel Medicare Advantage.
  3. Enroll in a standalone Part D drug plan during the same window.
  4. Leave Medicare Advantage so your Original Medicare and Medigap coverage start the first of the month, usually January 1 if you switch during the fall window.
  5. Keep copies of your confirmations and check that your effective dates line up with no gap.

Expert insight

“You can switch from Medicare advantage to a Medicare supplement. This is actually a very common question , and there is something very important to know . Although yes you are allowed to switch most insurance companies require you go through underwriting meaning you are approved or denied based on your current health status. BlueCross is the only company that WILL NOT underwrite if you are a current Blue member for the life of the members policy. This is a huge benefit no other carrier offers.”

Frequently Asked Questions

Can I switch from Medicare Advantage to Medigap?

Yes. You can leave Medicare Advantage during the Annual Enrollment Period (October 15 to December 7) or the Medicare Advantage Open Enrollment Period (January 1 to March 31). Buying a Medigap policy is a separate step that may involve health questions unless you qualify for a guaranteed issue right.

Can you switch from Medicare Advantage to a Medicare Supplement without answering health questions?

Only if you have a guaranteed issue right, such as the 12-month trial right or your plan leaving your area. Outside those situations, a Medigap insurer in North Carolina can use medical underwriting.

When can I switch from Medicare Advantage to Original Medicare?

During the Annual Enrollment Period, October 15 to December 7, with coverage starting January 1, or during the Medicare Advantage Open Enrollment Period, January 1 to March 31.

Do I need a Part D plan if I switch to a Medigap policy?

Yes. Medicare Supplement plans do not include drug coverage, so you need a standalone Part D plan to stay covered and avoid a late enrollment penalty.

Can I be denied a Medicare Supplement plan?

Outside a guaranteed issue window, a Medigap insurer can decline you or charge more based on your health. Getting approved before you drop Advantage is the safest approach.

What is the Medigap trial right?

If you joined Medicare Advantage when first eligible, or dropped a Medigap policy to try Advantage, you can switch back to Medigap on a guaranteed issue basis within your first 12 months, with no health questions.
