ACA Open Enrollment 2027 in North Carolina: Dates and Deadlines

If you buy your own health insurance in North Carolina, open enrollment is the window when you can pick or change your plan for the year ahead. For 2027, the dates were uncertain for a while, but they are now confirmed: open enrollment runs November 1, 2026, to January 15, 2027.

Here are the dates and deadlines that matter, what changed, and what to do before the window closes.

When is ACA open enrollment for 2027?

Open enrollment for 2027 Affordable Care Act coverage in North Carolina runs November 1, 2026, to January 15, 2027. North Carolina uses the federal marketplace at HealthCare.gov, so these are the dates that apply across the state. During that window you can enroll in a new plan, switch plans, or renew the one you have.

You may have seen conflicting deadlines because a 2025 federal rule sought to shorten the enrollment window beginning with 2027 coverage. Following federal court rulings affecting marketplace rules, CMS confirmed that open enrollment on HealthCare.gov will run November 1, 2026, through January 15, 2027. These are the currently published dates.

December 15 deadline for January 1 coverage

One date inside the window matters most: December 15, 2026. To have your coverage start January 1, 2027, select a plan by December 15 and pay any required first premium by your insurer’s deadline. If you select a plan between December 16 and January 15, your coverage generally starts February 1, again once any required first premium is paid. December 15 is the date to circle if you want no gap at the start of the year.

If you already have a marketplace plan and do nothing, you will usually be auto-renewed into the same or a similar plan for January 1. That is convenient, but it is not always your best option, which is why a quick review before December 15 is worth the time.

ACA open enrollment vs. Medicare Annual Enrollment

These two run at the same time of year and are easy to mix up, but they are for different people. ACA marketplace coverage is for people who buy their own insurance and are generally under 65. Medicare is for people who are 65 or older, or who qualify through a disability.

Medicare’s Annual Enrollment Period, the time to review or change Medicare health and prescription drug coverage, runs from October 15 to December 7, and we cover it in our post on the Medicare Annual Enrollment Period. If you are shopping for an individual or family marketplace plan, the ACA dates above apply. If you have Medicare, the Medicare enrollment rules and dates apply, even if you are under 65.

What to check before you renew for 2027

Auto-renewal is convenient, but it can cost you. Before your plan rolls over for 2027, take a few minutes to review your options and update your marketplace application, including your income and household details. A quick check now can prevent a surprise premium or a dropped doctor in January.

Before you renew, review these five things:

  1. Your doctors and hospitals are still in the plan’s network.
  2. Your prescriptions are still covered on the plan’s drug list.
  3. The new premium, compared to other plans available to you.
  4. Your income estimate, so any subsidy is accurate.
  5. The deductible and out-of-pocket maximum for the coming year.

Whether you qualify for help paying your premium can change from year to year, based on your income and the plans in your area. You can check your options in our post on subsidy eligibility, or review individual and family coverage in North Carolina to see how marketplace plans work.

Missed open enrollment? Special enrollment periods

If you miss the January 15 deadline, you generally cannot buy a marketplace plan until the next open enrollment, unless you qualify for a Special Enrollment Period. Certain life events open this window so you can enroll outside the normal dates.

You may qualify for a Special Enrollment Period after certain life events, such as losing qualifying health coverage, getting married, having or adopting a baby, or making a qualifying move. Divorce or legal separation may qualify you if it causes you to lose health coverage, and eligibility requirements depend on the event. Depending on the event, you generally have 60 days before or after it to enroll, and some situations have different deadlines, including up to 90 days after losing Medicaid or CHIP coverage. A licensed agent can confirm whether you qualify and help you enroll in time.

Expert Insights

“One of the most common questions we receive during ACA Open Enrollment is whether members need to call in to process a renewal. While many plans can auto-renew, we strongly recommend reviewing your coverage with your licensed agent each year. In many cases, simply updating and submitting your renewal for the new plan year can result in lower monthly premiums or better plan options, even if your income, household size, or healthcare needs have not changed.”
